For tracking and accounting purposes, we have a two step enrollment process. First you will complete the enrollment form below to enroll with Finance Your Care. After you do, you will be directed to click on a link to complete the lender platform enrollment with Credit Technologies. Make sure you complete both forms to get enrolled. The lenders may take up to 10 business days to complete your approval.
