We have a two-step enrollment process:

For tracking and accounting purposes, we have a two step enrollment process. First you will complete the enrollment form below to enroll with Finance Your Care. After you do, we will send you an email with a check list of items that will be required by our lender platform. A copy of those required documents is listed below the enrollment form in a PDF format that you can print out. We will not be able to enroll your practice with our lenders until all the documents required have been provided.
